Onsite Health Center

Medical Director Liz von Wellsheim, GNP provides basic medical services and ongoing medical management for residents. She works closely with residents' primary care physicians to help manage each patient's dementia and general health needs.

In addition to seeing patients in our onsite Health Center and in our campus homes, Liz offers house calls to private homes or other care centers to assist in creating solutions for issues arising from memory loss.

To provide seamless care across the healthcare continuum, we use an Electronic Medical Records program to communicate patient information with area physicians and hospitals. So when you arrive at Sacred Heart Medical Center, for example, your updated medical record is already there!

For check-ups and minor or chronic ailments, residents never need to leave the ElderHealth & Living campus.
